ABSTRACT

@#The microbiologist, who aged 44 man has work with glander DNA extraction between January and March at 2022, was developed sumptoms with fever, headache, muscle pain, weakness, cut throat, cough at 4 March, 2022. On March 7, he had tested Covid-19 and the result was negative. He was given 1gr tefazoline by eight-time interval for two days. Despite completing the therapy, episodes of fever and headache increased. A medical evaluation, which included MRI test was no disorder was developed. On March 12, painful with leg and developed muscle pain. He continued to difficulty to walk and cough, fever and weakness. On March 13, he has admitted hospital with diagnoses pneumonia. </br>He had continued sign with pneumonia in both lung, fever, infiltration with right leg, cough, headache, and glandule node in hospital. By PCR test, glander DNA was detected in sputum in National Center for Zoonotic Diseases laboratory. He recovered 20 days in hospital. </br>He has 12 days incubation period and infection route was by worked with glander strain and it was pneumonia form with laboratory-acquired human glanders. </br>Human glander case is rare in Mongolia. Three human glander cases had registered in 1966, 1972, 1977 among prison’s horse herder in Mongolia.

ABSTRACT

Purpose@#Researchers suggest that the prevalence of infertility varies between developing and developed countries, with differences in infertility care, socioeconomic status, lifestyle, and reproductive disorders such as pelvic inflammatory disease and sexually transmitted infections being the main risk factors. The research project aims is to define risk factors for female fertility in the Mongolian population. @*Methods@#This study was conducted between 2016-2018 using a cross-sectional survey of analytical research. Participants were randomly selected from Ulaanbaatar and the Central, Western, Eastern, and Khangai provinces according to Mongolia’s regional geographic model. The contents of a questionnaire were comprised of 5 units with 95 questions including socio-economic, geographical, lifestyle, health education, reproductive health indicators, sexual behavior. General physical characteristics were measured according to the standard. @*Results@#The prevalence of the female fertility rate in the Mongolian population is 7.4%. Female participants were classified into 2 groups, namely infertile and fertile, and we developed a case-control study. Among the socio-economic factors influencing infertility, primary education aOR: 1.6 (95% CI 0.98-2.66), monthly household income lower than the average aOR: 1.1 (95% CI 0.77- 1.66), living in rural areas OR: 2.3 ( 95% CI 1.46-3.68) were crucial risk factors. As for reproductive and general health indicators, STIs aOR: 1.8 (95% CI 0.98-3.50), especially gonorrhea OR: 2.8 (95% CI 1.14-6.91), and thyroid disorders OR: 1.7 (95% CI 1.03). -2.97), grade 3 obesity OR: 3.8 (95% CI 1.05-13.95) are estimated risk factors for infertility.@*Conclusions@#Of all potential socio-economic factors, residence status, education and financial situation are significant for female infertility meanwhile reproductive health indicators include sexually transmitted infections, thyroid disease, and obesity.

ABSTRACT

Objective: This study examines the job satisfaction of Mongolian hospital nurses by comparing their status and workload of Mongolian nurses with Japanese nurses’ one.Settings and participants/Methods: Survey data were collected from randomly selected 200 nurses (100 were Mongolians and the other 100 were Japanese) who agreed to participate in the survey. Data were collected through a self-administered survey questionnaire. Survey items were age, the duration of work experience, work position, health condition, accumulated fatigue, stress level, and whether having family members who need child care or nursing care. Collected data were analyzed by t-test and Wilcoxon Rank Sum test.Results: The average age of Mongolian nurses was significantly lower than that of Japanese nurses. Consequently, the average work experience of Mongolian nurses was less than that of Japanese nurses. More Japanese than Mongolian nurses had family members in need of care. Job satisfaction and status were significantly higher among Japanese than Mongolian nurses. However, Japanese nurses have family members who need child care or nursing care at a higher rate than Mongolian nurses. Job satisfaction of Japanese nurses about their work and job status was significantly higher than Mongolian nurses’ one. However, the opposite result was found in the job satisfaction about their workload. The influence of the relationships among nurses on the job satisfaction was significantly greater in Mongolian nurses than in Japanese nurses. Job satisfaction of Japanese nurses about their salaries was significantly higher than Mongolian nurses’ one.Conclusion: In order to raise nurses’ job satisfaction in Mongolia, it is necessary to raise their “occupational status” and salary of nurses.

ABSTRACT

Introduction@#Endothelial progenitor cells (EPC) have a role in the maintenance and promotion of vascular repair and are negatively correlated with coronary atherosclerosis. @*Goal@#To culture of EPC-CFUs during coronary atherosclerosis, evaluate endothelial nitric oxide synthetase (eNOS) enzyme levels in the culture.@*Materials and Methods@#The 10 ml blood was drawn from the peripheral vein of 12 man patients that stable angina 4, acute myocardial infarction (AMI) 4 and healthy people 4. Peripheral blood mononuclear cells were isolated by Ficoll density-gradient centrifugation and EPC-CFUs was assayed after two platings and a 6 day culture on fibronectin coated, 72 well plates, as described. eNOS enzyme titers were determined by ELISA according to the protocol in the cells culture.@*Results@#The people were 52±2.12 years. The number of EPC-CFUs increases with accordance of patients with stable angina, AMI, healthy people with the statistical significance (F=17.3, p<0.001): stable angina (2.6±0.47 colony/well), AMI (6.7±0.81 colony/well), healthy people (10.5±1.34 colony/well). Furthermore, ANOVA test of eNOS enzyme levels in patients with stable angina (5.2±0.61 pg/ml), AMI (8.7±1.49 pg/ml) and healthy people (13.7±2.48 pg/ml). The significant difference (F=6.2, p=0.003) was observed among the three groups. The number of EPC-CFUs had direct significantly correlation (r=0.621, p<0.001) with the eNOS enzyme levels of this culture.@*Conclusion@#Number of EPC-CFUs and eNOS enzyme levels decrease at patient with stable angina, indicate more than endothelial dysfunction.@*Ethical approval@#The ethics committee of Mongolian National University of Medical Sciences (ID: 6/3/201506, approved on Jan 01, 2015)

ABSTRACT

@#BACKGROUND: According to the World Health Organization (WHO), 10-15% of couples of reproductive age have infertility. According to researcher D. Sukhe (1999), hormonal infertility in the reproductive age of women was 33.6%, which was a large part of the cause of infertility. In recent years, the number of cases of endocrine disorders, including malignancy and sexually transmitted infections, has been increasing year by year. According to WHO reports, thyroid disorders have a prevalence of 49.3% for active reproductive age (30-50). According to the report of the Health Development Center in 2016, since the thyroid disorders are the second most common disease in endocrine gland disease, our study found that the infertility in reproductive age of women can be substantial due to the loss of thyroid gland. METHODS: The study was carried out by the couple of 20-45 year-olds and modeled as an analytical study model. The questionnaire was used for the couple’s interviews and some of the measurement of body and serum use of TOSOH Corporation AIA-360, Tokyo, Japan. On the serum, anti-TPO and аnti-TG carbohydrates are identified by the Cobas e-411 analyzer under the manufacturer’s accompanying protocol. RESULTS: 76.7% of women were diagnosed with infertility euthyroid, 0.7% hyperthyroidism, 22.6% hypothyroidism (3.8% with overt hypothyroidism and 18.8% subclinical hypothyroidism). Prevalence of TAI, in 6.7% isolated positive anti-Tg were found, and 14.3% had isolated positive TPO, In 3.7% of cases, both types of autoantibodies were present. We analysed binary logistic regression for anti-TPO and anti-TG autoantibody in the positive and negative group in past obstetrics history, evidence of positive of anti-TPO and anti-Tg was increased risk of miscarriage 2.2 times (OR = 2.2, p <0.01). CONCLUSIONS: Women with disorders in our study have high percentage of subclinical hypothyroidism and have higher rate of thyroid autobodies in serum which may be a problem for women with infertility and pregnancy complications due to the loss of thyroid gland. There is a need to develop a principle of recovery and treatment.

ABSTRACT

To determine effects of shift work on nurses’ health by arterial hypertension and diabetics type 2. The design of this study is cross sectional design. Subjects were 634 nurses who were chosen from 5500 nurses undertaking rotating shift work by random sampling method in accordance with criteria of research recruiting. The 64.5% of research participants were from Ulaanbaatar city and 35.5% from rural areas. Average aging is between 40.2±19.8 (p<0.0001). Three hundred three of the participants (50.8%) were night shift nurses and 293 (49.2%) day shift nurses. Obesity distribution was through trunk area at 93%. The participants’ 33.6% has high blood pressure; and 12.3% are at medium risk of diabetics and 4.2 % at high risk for near 10 years. According to test of fasting blood glucose in capillaries, 28.3 % of the nurses who are involved in the research has changes on their fasting glucose level and 14.5% has diabetics. As seeing from lab test and body index, shift nurses are at high risk of arterial hypertension and diabetics and these diseases are more common among the nurses.

ABSTRACT

Background. Acute alcohol delusions of persecution usually show up delusions of persecution, which is begun auditory hallucination, illusion, fair, anxiety in night time. The patient may conduct to attempt suicide or social harmful activities from the fair and the anxiety. Therefore, we studied types of delusion in alcohol related psychosis and risks of social harmful activities due to delusion. Aim. To identify types of delusion in alcohol related psychosis, risks of social harmful activities due to delusion.Materials and Methods. The study was conducted on the 110 cases of suicide (100 cases among males and 10 cases among females). In the study used special questionnaire with 45 chapters and 431 questions. The current mental condition was identified by the objective and subjective history of patients with Michigan scale questionnaire.Results. We were compared forms of alcohol related psychosis to delusion symptoms, there were occurred 29.6% of 64 patients with delusions of persecution during withdrawal state with delirium without convulsions (F10.40) and 34.4% of 64 patients with delusions of persecution during predominantly delusional psychotic disorder (F10.52). Those patients in 37 case (or 33.6%) were with suicidal thinking and 20 patients (or 18.2%) made suicidal attempts. 23.4% of 64 patients with delusions of persecution, 24.07% of 54 patients with hallucinatory delirium made suicidal attempts.Conclusions. Delusion symptoms in alcohol related psychosis are belonged to clinical form and were showed up many types.

ABSTRACT

BackgroundThe preparations of multi-component have been the subject of chemical study for a long time. Therefore, when compounding the preparations of multi-component in traditional medicine, their taste is cautiously relied on, as the power of the one medicine should not be subdued with the power of another. Additionally the properties of the components and their regulating effects on the body systems are also considered. Our research group has been carrying out tests for raw materials,which are contained in multi-component preparations. However, it is a necessity to conduct phytochemical study on multi-component preparations in order to isolate pure biological active compounds and to identify their structure as well as to quantify its amount by modern techniques of analysis.GoalThe aim of the present study was to isolate pure biological active substances from Mongolian traditional medicine Garidi-5 and to elucidate their structures, which was used in Mongolian traditional medicine for the treatment of inflammation and as a pain relieving remedy.Objectives:1. To isolate pure substances from Garidi-5 and carry out tests to identify and determine their structure2. To quantify the amount of biological active substances.Materials and MethodsMongolian traditional medicine Garidi-5 has been selected as a biological natural product for the study. Garidi-5 is a traditional Mongolian medicine consisting of 5 medicinal herbs, namely Terminalia chebula Retz., Aconitum Kusnezoffii Reichb., Acorus calamus L., Saussurea lappa L., and musk of Moschus moschiferus and manufactured in the Drug factory of Traditional Medical Science Technology and Production Corporation of Mongolia. In this research, in order to determine the total content of phenolic compound was used the Folin–Ciocalteu method, which based on performing dark blue color complex compound. Isolated substance identification was determined by the TLC, UV and IR spectrophotometric methods. Inaddition it was checked melting point of the isolated substance. Determination of Gallic acid30g Garidi-5 was macerated in 60ml 80% methanol at room temperature for 24 h. After extraction, the extract was concentrated and vacuum evaporated. Different solvents from hexane, chloroform, ethyl acetate and n-butanol were used for theexperiment. All the extracts collected, evaporated and chromatographed on Silica gel column. Future purification of active fractions on Silica gel with methanol yielded the compound G1 which was further characterized as Gallic acid. Total phenolic content was determined spectrophotometrically according to the Folin–Ciocalteu’s method with slight modification. Gallic acid was used as a standard phenolic compound. Briefly, 1 ml of extract solution contains 1 mg extracts, in a volumetric flask diluted with distilled water (46 ml). One ml of Folin-Ciocalteu reagent was added and the content of the flask mixed thoroughly. After 3 min, 3 ml of Na2CO3 (2%) was added and then the mixture was allowed to stand for 2 h with intermittent shaking. The absorbance was measured at 760 nm in a spectrophotometer. All measurements were performed in triplicate.ResultsIn this research, TLC method on silica gel plates was used in order to identify the biological active pure substances from Garidi-5. Preliminary TLC experiments indicated the presence of Gallic acid in Garidi-5, which was isolated by column chromatography by comparing with reference standard substance (Gallic acid). Gallic acid was determined in the solvent system benzole-ethyl acetateformic acid- acetone (5:5:2:0.5) in isolated substance (G1). It showed blue color, Rf =0.65, on TLC plate. [1] For the characterization of two samples it was carried out IR analysis for each. In the IR spectra of G1 and standard substance can be recognized by the following absorption frequency regions: 700-900 cm-1 for Car-H; 1000-1300 cm-1 for vibration of bonds in various oxygen containing groups, 1350-1470 cm-1 for vibrations of –CH, -CH2 and –CH3 groups; 1500-1630 cm-1 for skeletal vibrations of aromatic rings, >C=O bonds; 2800-2950 cm-1 for stretching vibrations of –CH, -CH2 and -CH3 groups in saturated aliphatic structures; and 3030-3350 cm-1 for stretching associated vibrations of -OH groups in aromatic rings and aliphatic structures. As a result it was revealed that both IR spectra of G1 and standard substances were similar. [3]Further for the characterization of two samples it was carried out UV analysis of each. In the UV spectra of G1 and standard substance can be recognized by the following absorption frequency regions: 260-280nm for benzole groups; 200-225nm for carbonic acids; 400-770nm >C=O bonds, which reveal the presence of Gallic acid. In addition, melting point of isolated substance G1 was analyzed and detected at 2410C, which was similar to the standard substance’s melting point. [4]Moreover, Mongolian traditional medicine Garidi-5 contains 24% of the biological active substance (total phenolic compounds). [2]Conclusions:As a result of current study on Mongolian medicine Garidi-5, it was isolated one essential substance from ethyl acetate fraction. The phytochemical analysis reveals the presence of Gallic acid in Garidi- 5, which was determined by thin layer chromatography, UV and IR spectrophotometric methods. Mongolian traditional medicine Garidi-5 contains 24% of the biological active substance. Thus, the isolation of Gallic acid from multi-component preparations and identification of its structure was first phytochemical study conducted in our laboratory.
